Important Details and Documents:

**Group registrations are due in the OEC by October 15, 2008**

(for you to copy and distribute to the teens in your parish/school - we marked the due date on the brochure as Wednesday, October 10, so you have a couple days to return the group registration form to us by Wednesday, October 15. Also, the registration includes a medical release form for you to keep - do not send it back – as liability protection);

The mailing form. (this is the only form parishes/schools need to send back... along with the registration fees... the form and payment is due to the Office of Evangelization and Catechesis by October 15 - NO walk-ins!); Note: before sending us the form and check, please FAX the form and a copy of the check to the Office of Evangelization and Catechesis at 608-821-3181;
