Mission Cooperative Plan - Participant Application

Thank you for taking the time to complete the following application for the Mission Cooperative Plan with the Diocese of Madison.  Applications are due by January 1st for consideration for next year's program.  If accepted, you will be notified by February or March.  Your visits must be conducted by the end of the calendar year.
