Registration by TBA is required for guaranteed participation. After TBA, participation may be subject to limitation.
Eligibility
The date of death must be 12 months or more before the date of committal.
At Resurrection Cemetery, the committal includes: Free placement in the Sacred Heart of Jesus crypt. Remains should be placed in a dignified container like a cremation urn. Urns with remains must be labelled on the outside with full name and date of death. After one year, depending on space requirements, cemetery staff may relocate remains another dignified location on Resurrection Cemetery's sacred ground. Bring cremated remains to the cemetery on the appointed day in an urn or a durable container of standard size. We may not be able to accommodate oversized containers. Urns are available for purchase from the cemetery. Recovery of remains: No charge for disinterment for one year. After one year, prevailing disinterment rates apply.
At Mount Olivet Cemetery, placement will be in the Pieta Ossuary. This placement is permanent, irreversible, and no option for recovery of remains is available. Because of this, the cemetery must receive a state-issued cremation authorization form prior to the committal. A separate waiver stating that these conditions are understood and accepted will also be required before participation. Remains must be placed in a bag made of a dignified fabric like silk or velvet. The name of the deceased, and their date of death, must be attached to the outside of the bag. If assistance is requested before TBA, the cemetery will provide the bag, label, and will assist with the transfer of remains, and can also assist with obtaining the necessary paperwork from the state.

Memorialization options vary depending on the cemetery, but can include an engraved brick, a small granite memorial, and a bronze plaque. Contact the office for details. Options vary from cemetery to cemetery, and not all options are available at every cemetery.
